DSO Application Process
This guide explains how to apply for adult developmental services funded by the Ministry of Children, Community and Social Services (MCCSS) through Developmental Services Ontario (DSO). It outlines the four key steps: preparing eligibility documents, applying online or by phone, completing the application with the support of a DSO assessor, and staying connected while waiting for services. The document details what proof is required for developmental disability, Ontario residency, and age, as well as what to expect during assessment meetings. It helps individuals and families understand the process and prepare for accessing essential supports.
DSO Housing Tool Kit
The DSO Housing Toolkit is a six-step library of resources designed to help individuals plan for their housing future. Developed by Developmental Services Ontario with input from people with lived experience, it includes tools like the “My Housing and Support Plan” form. The steps guide users through identifying housing needs and preferences, exploring location and housing options, planning finances, managing supports, adapting to change, and learning from others’ experiences. Each section features expert advice, tools and resources to fill gaps in your plan. Local DSO offices are available to help with navigation and supplemental support.
Ontario Powers of Attorney Kit
This official kit from the Ministry of the Attorney General provides step-by-step instructions and forms for creating a Continuing Power of Attorney for Property and a Power of Attorney for Personal Care. It helps Ontarians plan ahead by appointing trusted individuals to make financial and personal care decisions if they become unable to do so themselves. The booklet includes clear definitions, guidelines, and legally valid forms under the Substitute Decisions Act, 1992.
Person-Directed Planning Guide
This resource explains person-directed planning, a process that helps individuals with developmental disabilities take charge of their future. It emphasizes building a meaningful life based on personal strengths, interests, and goals, rather than focusing on limitations. The guide outlines how to create a vision, identify supports, and develop an action plan that reflects what matters most to the individual. Families, friends, and community members play a supportive role, while the person at the centre makes key decisions. The document encourages independence, choice, and community inclusion, offering a practical framework for planning a fulfilling and self-directed life.
STEPS to Independence Guidebook
The STEPS to Independence guidebook helps individuals with intellectual disabilities assess their readiness for semi-independent living. It covers seven key life skill areas: health, household management, self-care, financial skills, community participation, social interaction, and academic/employment abilities. By identifying current strengths and areas for growth, the guide provides a framework for building confidence, setting learning goals, and planning next steps. It can be completed by the individual, family, or support network, and serves as both a self-assessment and conversation starter. The guide fosters independence by highlighting progress and supporting skill development for future success.
Transition to High School: A Parent Guide
This guide, developed by the Toronto Catholic District School Board in partnership with Surrey Place Centre, helps parents support their child’s transition from Grade 8 to Grade 9, with a focus on students with Autism Spectrum Disorder (ASD). It provides practical tips on preparing for new routines, building independence, and strengthening social, communication, and organizational skills. The resource outlines different course levels, strategies for managing stress, and suggestions for collaborating with teachers and school staff. It also includes planning checklists and advice on accessing supports, ensuring families are better prepared for a successful high school experience
